If adequate correction of a fixed hammertoe deformity cannot be achieved with PIP joint resection arthroplasty, additional procedures are necessary. First, extensor tenotomy is performed at the MTP via a dorsal stab incision. Releasing both the EDL and extensor digitorum brevis is important. If correction remains inadequate, release of the dorsal MTP joint capsule is performed through the same stab incision.
